Student Referral & Reporting
The Dean of Students Office provides many avenues for reporting student concerns. Please use the reporting forms below to report student concerns. We will make sure the most appropriate venue for support is provided.
If the student’s behavior is clearly and imminently reckless, disorderly, dangerous, or threatening, including self-harm behavior, call 911 or Campus Police: (415) 338-7200. If you are calling 911 from a cell phone on campus, dial (415) 338-2222 instead for more direct access to UPD.
Action Care Team - Student of Concern
The Action Care Team (ACT) is comprised of a team of SF State professionals committed to the health, safety, and well-being of students within the campus community. This team receives referrals and information pertaining to students of concerns, collects information, and identifies or enacts appropriate strategies for addressing potentially threatening situations. The purpose of this team is to provide a means for early intervention of at-risk students through collaborative work of appropriate faculty, staff, and students.
Our focus is assisting students who may be in distress. We coordinate resources and implement a centralized response, with the goal of providing the student the support they need and keeping the SF State community healthy and safe.

Office of Student Conduct
The Office of Student Conduct at San Francisco State University is charged with upholding the Student Conduct Code, which identifies behavioral expectations for all students. If you have knowledge of a student violating the Student Conduct Code, you are encouraged to notify our office of the incident by completing Office of Student Conduct Reporting Form (see link below).
To report allegations of student misconduct, use this form. You may report any non-academic incident(s) that happened on or off campus. The Office of Student Conduct may investigate the incident(s) to determine if a violation has occurred and to determine appropriate outcomes.
When completing this form, please include your name, title, and contact information. Please note that anonymously submitted forms may limit our effectiveness in dealing with concerning behavior.
Due to privacy rights, the outcome of a student conduct matter will not be shared with the individual who reported the allegation. For more information, please visit the Office of Student Conduct website.

Dean-on-Call Referral for Student Support
The Dean-On-Call program is housed within the Dean of Students Office and works in collaboration with our campus partners to support the holistic experience of our students. At times students may experience challenging life events, extenuating circumstances that may distract them from important academic work inside the classroom, or may generally need assistance in navigating the competing demands of being a student at SF State. Our goal is to assist each student with the support and resources they may need to fully engage in the academic setting and progress toward graduation.
If you have a concern about a student that may not fit in the crisis or student misconduct realm, we encourage you to submit a referral form. You may also elect to recommend the student visit the Dean-On-Call Drop-In hours in Student Services Building (SSB) 403, Monday through Friday between 8:00am - 5:00pm to speak directly to one of the Deans-On-Call. Title IX and DHR
All students have the right to participate in SF State programs and activities free from discrimination, harassment, and retaliation. SF State prohibits discrimination and harassment on the basis of gender, which includes sex and gender identity or expression, or sexual orientation. The University also prohibits any behavior that constitutes sexual misconduct, which includes any sexual activity engaged in without affirmative consent, where affirmative consent means an informed, conscious, voluntary, and mutual agreement to engage in sexual activity.
After filing a report, a member from the Equity Programs & Compliance Office will follow-up with the Complainant to provide information on rights and options. For more information please visit the Title IX website.
